The OPs car has some problem for sure, but every car I've had since the advent of electric fans instead of the old fashion fan running of the drive shaft had a warning sticker under the hood that the fan could come on if the car was hot. My 328d used to from time to time indicating it had probably gone through a DPF regen cycle.
