The cause: about 100K miles prior, 5th gear had come off. The repair performed involved a thrust washer. The thrust washer came off, went through the gear case and was caught on the magnet on the bottom of the tranny case. When it went through the case, it chiped some teeth and misalighed 5th and the shift fork.
